- Coventry/Southcare/First Health (National, Leased, HMO) - No out-of-state plans
- Great West
- Humana (Choice Care Network)
- Kaiser through PHCS (group plans only thru PHCS - no HMO group and no individual plans
- Lifewell Health Partners
- Medical Partners of America (MAG Mutual)
- Multi Plan
- PHCS
- Unicare
- Liberty Health Share
- UHC
If you do not see your insurance plan listed, please call our office. The managed care health insurance market changes rapidly, and specific coverage questions should be discussed directly with your insurance company or our knowledgeable staff.
Insurance Plans Not Accepted
We do not accept the following insurance plans:
- Ambetter
- Amerigroup
- Kaiser (INDIVIDUAL PLANS or HMO Group)
- Medicaid
- Peachstate
- Tricare/Champus
- United Healthcare COMPASS
- United Healthcare "Core Network"
